MSD and its affiliates (collectively, “Sponsor”) seeks your written consent below to use and disclose your personal data as is described herein. Uses of your personal data would include certain internal processing purposes at Sponsor. Disclosures of such data would include its transfer to certain regulatory agencies and to the public through registry postings on the Internet. Such data collected up to the date of this consent will continue to be used and disclosed as is described herein. You have the right to access, correct and amend your personal data on file at any time. If you wish to do so, please, contact your Sponsor representative.

These uses and disclosures relate to the conduct of clinical studies in which you participate.

Specifically, Sponsor intends to use and disclosure your personal data as is described below.

Personal data (such as your full name and physical address):

1. Will be stored in electronic databases at Sponsor;
2. Will be used and disclosed to Sponsor’s representatives for the purposes of monitoring a clinical and non-clinical study and its conduct, and to determine potential investigators for future studies;
3. Will be disclosed to country and local regulatory agencies, as appropriate, to fulfill regulatory reporting obligations with such agencies; and,
4. Will be transferred to the United States for sites located outside of the United States, or to other countries, as appropriate, for purposes of the above uses and disclosures.

Certain data (such as your study site phone contact information and site location):

1. Will be posted on public registries in which Sponsor participates;
2. Will be used to contact you, if you are designated by your site to be the study site contact for prospective study subjects or for referring physicians who may contact the site regarding a study; and
3. Will be transferred to the United States for sites located outside of the United States, or to other countries, as appropriate, for purposes of the above uses and disclosures. Some of these countries may not offer the same level of privacy protection as you are used to in your country

By signing this form, you consent to Sponsor’s uses and disclosures of your personal data as is described herein. You may decide to revoke this consent; however, such revocation will apply only to new uses and disclosures relating to your personal data.

Please do not enter the personal data of others on this form, and also do not submit to Sponsor any personal data of site personnel prior to their providing consent by signing below.
